Individual and Family Plans:
Blue Solution PPO
Our most comprehensive plan, Blue Solution PPO is a traditional PPO with 4 different deductibles from which to choose -- $750, $1,500, $3,000 and $5,000.
BlueChoice PPO
BlueChoice PPO offers more deductible choices than any of our other plans — from $500 to $25,000. With BlueChoice PPO, you also have the opportunity to choose the amount of coinsurance you will pay and the option to pay only copays for doctors’ visits.
HSA Blue PPO and HSA Blue PPO PLUS
HSA Blue PPO and HSA Blue PPO PLUS are our HSA-compatible health insurance plans. One of these federally-qualified high deductible health plans is what you’ll need to take advantage of the tax savings associated with a Health Savings Account (HSA). The two plans, which feature deductible options, are identical except HSA Blue PPO PLUS covers prescription drugs, once you’ve met your annual deductible.
Access Blue PPO
Access Blue PPO is a limited benefit policy designed to protect you against the high costs associated with more serious medical care. In exchange for a lower monthly premium, you’ll have coverage for inpatient and outpatient services, radiation therapy, chemotherapy, dialysis and more.
Short-Term Blue
Short-Term Blue is designed to provide coverage for those who want short-term protection against catastrophic events such as unexpected illnesses, diseases and accidents. You choose the deductible -- $500 or $1,000 — and the number of days (a minimum of 30 and a maximum of 182) you’ll need coverage.

Traditional Fee-for-Service Plan

Our Group BasicBlue health insurance policy provides essential coverage for your employees at a much lower cost to you. This health plan provides a $1 million lifetime maximum benefit for covered services and coverage for benefits such as:
- Inpatient and outpatient hospital care
- Physician services for inpatient and outpatient hospital care
- Catastrophic major medical coverage
- Ambulance services
- Chemotherapy and radiation therapy
- Durable medical equipment
- Physical, speech and occupational therapy
Coverage for physician office visits and prescription drugs may be added.
You can enhance your employees' Group BasicBlue coverage with two important riders:
- The physician office visits rider extends their coverage to include physician office visits with a $35 copayment.
The prescription drugs rider adds valuable prescription drug benefits to their coverage.
Even by adding these two options, the cost for total coverage is less expensive than most major medical plans.
Employees may choose any doctor or hospital.
With Group BasicBlue, your employees are not required to choose physicians, specialists and hospitals from a predetermined list. They have the freedom to use any health care provider.
However, they can save money by selecting a health care provider listed in The Blue Book, a directory of health-care facilities, physicians and other medical professionals who have agreed to offer their services at predetermined fees. Employees may choose any doctor or hospital.
Also, participating providers will file claims for you, so there is less paperwork and less hassle.
